Introduction: Breast cancer is one of the most common types and its conventional treatment can cause important musculoskeletal and psychosomatic complications. The Pilates Method is shown as a strategy capable of mitigating adverse health symptoms and producing benefits on these aspects in this population. Objective: Systematically review the effects of the Pilates Method on musculoskeletal and psychosomatic disorders in individuals with breast cancer. Method: Systematic review and meta-analysis of randomized clinical trials carried out in the databases Embase, Scopus, LILACS, Web of Science, Cochrane Library, Medline, PEDro and SciELO, using the descriptors “Breast Neoplasms” and “Pilates Method”. Of the 202 articles found, 10 were included in this study and six were analyzed in the metanalysis for the outcomes pain, lymphedema of the affected limb, range of motion of the affected shoulder, handgrip strength, functionality and anxiety symptoms. The methodological quality of the studies was analyzed by the PEDro Scale and the metanalysis performed by the Review Manager software version 5.4 (Cochrane Collaboration). Results: The Pilates Method group showed superiority over the control group or minimal intervention in the lymphedema of the affected limb, in the flexion movements, external rotation and total amplitude of the affected shoulder and in the symptoms of anxiety (p≤0.05). This difference was not observed in pain, handgrip strength and functionality (p>0.05). Conclusion: The Pilates Method has better effects on lymphedema of the affected limb, range of motion of the affected shoulder and anxiety symptoms compared to control or minimal intervention in individuals with breast cancer.
